Event Request Form

Complete this form to request space for your MBA associated church or partner organization to host an event here at the Great Commission Center. Please read our Building Usage Guidelines before filling out the form below.

Due to churches meeting at the GCC every Sunday, groups may not arrive until after 8pm on Sunday evenings.

Fees may apply. Limited space is available. Usage is based on approval from Director of Missions and Ministry Coordinator

Building 1, Lower Level Location Request:

Washer and Dryer usage is only available with APPROVAL from MBA Staff. Usage fees may be charged to groups using the machines.

I/We (The Ministry, Partners, Participants, etc.) understand that the equipment on MBA premises and equipment belonging to MBA is to remain on MBA premises. The equipment is NOT to leave the premises unless given prior permission by the Director of Missions AND the Ministry Coordinator. Items leaving premises MUST be checked out on our log and ministry partners must leave a copy (digital or physical) of their Certificate of Insurance (COI) with MBA. *

Event Requestors/Users agree to hold harmless, indemnify and defend Metrolina Baptist Association (including Metrolina Baptist Association agents, employees and representatives) from any and all liability for injury or damage including but not limited to bodily injury, personal injury, emotional injury or property damage which may result from any person using the above described premises, its entrances and exits and surrounding areas, for User's purpose, regardless of whether such injury or damage results from the negligence of Metrolina Baptist Association (including Metrolina Baptist Association agents, employees and representatives) or otherwise. ACCEPTANCE: Upon Approval by Metrolina Baptist Association and by the signing of this contract, I am agreeing to the terms listed above. Upon receipt of the deposit, Metrolina Baptist Association agrees to reserve the Great Commission Center for the above dates and times. *
